well just to be clear i hate beyonce, i think she's a fucking idiot, but i think the dent is from fishing line holding the dress over her shoulders, and it just doesn't show up in the picture. this would explain the shape of the top hemline thingy of the dress as well...but not the discoloration of her skin around that area.
